What Is Peroneal Tendinopathy?

Pain and inflammation on the outer ankle from overuse

Peroneal tendinopathy refers to irritation or damage to the peroneal tendons, which run along the outside of the lower leg and ankle and attach to the foot. You might also hear it called peroneal tendonitis or tendinosis, depending on the stage of the condition.

There are two key tendons involved—Peroneus Longus and Peroneus Brevis. They follow a similar path down the leg, passing behind the bony bump on the outside of the ankle (lateral malleolus), but attach to different parts of the foot.

This condition is usually caused by overuse or repetitive movements that put too much strain on the tendons—especially those that involve rolling the foot outward. Over time, this extra load can irritate the tendons and surrounding tissue, leading to pain and swelling.

Contributing Factors

• Previous ankle sprains or chronic ankle instability
• Weakness in the lower leg muscles
• Poor foot alignment or biomechanics
• Unsupportive footwear
• Repetitive activities like running, dancing or basketball
• Improper training techniques

Common Symptoms

• Pain or tenderness on the outside of the ankle
• Swelling or warmth in the affected area
• Discomfort that worsens with activity or when rolling out the foot
• A feeling of instability around the ankle

Treatment Options

Reducing pain and supporting long-term recovery

• RICE (Rest, Ice, Compression, Elevation) – Especially important in the early stages
• Supportive Footwear – Helps reduce stress on the tendons
• Orthotics – To improve alignment and offload pressure
• Rehabilitation Program – Stretching and strengthening exercises to support healing
• Ankle Bracing or Strapping – For added support during activity
