What Is Tibialis Posterior Tendon Dysfunction (PTTD)?

A leading cause of adult-acquired flat feet

The tibialis posterior tendon is the key structure supporting the arch on the inside of your foot. When this tendon becomes overworked and weakened, it can no longer hold up the arch properly, leading to a condition known as PTTD or Adult Acquired Flat Foot.

This condition usually develops gradually over time from overuse—such as walking or running without proper support—and can worsen if left untreated. Poor foot biomechanics, sudden increases in activity, or wearing unsupportive shoes can all contribute.

Common Symptoms

• Pain and swelling behind the inside of the ankle
• Arch pain and visible flattening of the foot
• Symptoms worsen with walking or activity and throughout the day
• Inability to do a single-leg heel raise on the affected foot
• Usually affects one foot, but both can be impacted
